Many patients still experience that their healthcare pathways lack coherence. There is insufficient coordination between the different services, and transitions can feel uncertain. Our mission is to create better continuity, ensuring that everyone receives a safe and efficient care journey—no matter where they are in the system.

Focus areas:

  • Map where patient pathways break down.
  • Explore the invisible work carried out by both patients and staff.

How do we create continuity in patient pathways?

Ideas for action:

  • Develop tools to map “breakdowns” in patient pathways through digital tracking, ethnographic observation, or AI-based summaries of care journeys.
  • Examine the invisible work carried out by patients, relatives, and frontline staff to create coherence across sectors.
  • Use registry data from primary care (GPs, physiotherapists) to identify gaps in care pathways.
  • Combine theories of cross-disciplinary collaboration with implementation research to break down silos. We need to understand how to truly create continuity.
  • Collaborate across sectors to explore how new approaches to governance or responsibility-sharing can make care pathways more coherent and ensure better handovers.
