Fragmentation & Poor Coherence

Many patients still experience that their healthcare pathways lack coherence. There is insufficient coordination between the different services, and transitions can feel uncertain. Our mission is to create better continuity, ensuring that everyone receives a safe and efficient care pathway — no matter where they are in the system.

Unrealised Potential of Individualisation​

We have the data, but turning it into personalized treatment tailored to the specific situation is still a challenge — especially in decentralized environments such as municipalities.

Unmet Needs for Self-Management​

When developing self-care tools, we need to consider design, communication, and equal opportunities. Standard solutions rarely fit everyone and often overlook those who need support the most.

Data Use & Ethics​

Health data holds enormous value, but also significant risks when systems do not work together and when ethical frameworks and patient trust are lacking.

Implementation as the Missing Link​

Promising pilot projects are rarely scaled up because implementation is often complex and ends up being deprioritized.

Educational Gaps in Digital Health​

Digital tools are not enough. Professionals and citizens need knowledge, mutual trust, and a strong learning culture.

Far too often, important perspectives are excluded from innovation processes — especially those of vulnerable patients, low-income citizens, or groups experiencing gender-related health inequalities.

The concept of a Digital Health Twin, a virtual copy of a person’s health, holds great potential. However, it requires a holistic approach, cross-disciplinary collaboration, and a clear plan to create real value.
